Pick-Up/Drop-Off COVID-19 Testing from OccuMed of New England:
OccuMed of New England has partnered up with the Broad Institute of MIT and Harvard to offer our customers fast and inexpensive COVID-19 tests. The Broad Institute, a genome sequence research institution, has responded to the COVID-19 pandemic in many ways, including clinical testing for COVID-19. Their novel automation system has allowed them to test large amounts of samples at a fast rate and low cost.
The Broad Institute uses the anterior nasal RT-PCR test to detect the present infection of COVID-19. Anterior nasal sampling is a minimally invasive test and results in less patient discomfort than sampling from other upper respiratory anatomical sites (Office of the Assistant Secretary for Health). The collection of anterior nasal swabs is less complex and can help reduce the risk of the spread of infection to the healthcare providers, by (1) reducing the duration of the procedure, and (2) allow the patient to perform self-administration while under supervision (Office of the Assistant Secretary for Health).
OccuMed of New England is aligned with the Broad Institute’s mission of improving the health of our communities by offering businesses and the public a chance to test their employees and families for COVID-19. Testing is a preventative service that can help to limit the spread of COVID-19, especially in the workplace. OccuMed is offering delivery services to help alleviate the responsibility of prepping and delivering test kits for customers.
Description of Pick-Up/Drop-Off Services (Important Information to Read):
This new service from OccuMed of New England allows patients to pick-up a requested number of test kits, bring them to their own desired location, self-administer the test, and drop-off the test kits at our location to be brought to the Lab for testing. This gives you the convenience to provide your business or family with a self-administered testing option in-house and relieves you of delivering samples to the Lab. This testing option is great for a small group of people (under 25 people). OccuMed of New England will pre-label test kits for the day of testing, provide you with nitrile gloves for the self-administration for the test, and instructions for the self-administration process. Clients will need to come in and pick-up the test kits at a scheduled time, and drop them off within 24 hours from when the sample was taken. The pick-up and drop-off location for the test kits is 5 Dan Rd, Canton, MA 02021. Once samples are collected, simply drop-off the completed test kits at the same location!
Use the calendar provided to schedule your time for pick-up. When selecting a time from the calendar, select the best time to pick up the test kits. You will not be allowed to schedule a time for pick-up with less than 24 hours’ notice. From there, you will need to insert all of the information for each patient needed for the testing process. This information includes a patients name, birth date, email, phone number, street address, ethnicity, and race. All of this personal identifying information is very important and MUST be provided. When you get to this step, you will be asked to download an excel sheet, fill in the information for all of the patients, save it, and then upload it. After, you will be asked to enter your desired drop-off time. Finally, you will make the purchase of the tests at the pick-up location. I understand that I am fully and personally responsible for my own safety and actions while and during my participation and I recognize that I may be in any case be at risk of contracting COVID-19.
I understand that an invalid/unsatisfactory (also known as a Test Not Processed (TNP)) result will not be refunded. An invalid/unsatisfactory sample results from the inability of the Lab to retrieve a conclusive test result due to the lack or excess of mucus, the tube is cracked or damaged, the sample is placed upside down in the tube, there is a laboratory accident, and more. I understand that an inconclusive test result will not be refunded. An inconclusive test result occurs when either (a) one of the two viral probes (either N1 or N2) being positive while the other is not detected after two attempts on the specimen or (b) results are discordant after two attempts on this specimen. It is recommended that the patient should consider obtaining a new test, but the patient will absorb the cost. A collection of multiple specimens from the same patient may be necessary to detect the virus. OccuMed of New England is not responsible for a test result being inconclusive. Each test, and its result, is provided for informational purposes only and is not a substitute for the care, medical advice or treatment provided by your physician.
The test results are subject to the same privacy and release restrictions that HIPAA (The Health Insurance Portability and Accountability Act of 1996) places on all medical records. The HIPAA Privacy Rule, protects the privacy of individually identifiable health information. Patients should be aware that test results will not automatically be forwarded to your physician or become a part of office medical records unless you make a specific request. Results will be emailed directly to you, only if the “Authorization to E-mail My Test Results” section of the Intake form is complete.
OccuMed serves as a collection service only. All tests will be reviewed by an OccuMed medical professional.
With full knowledge of the risks involved, I hereby release, waive, discharge OccuMed of New England, an EquipNet Company, its board, officers, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained while participating in any activity while in, on, or around the premises or while using the facilities that may lead to unintentional exposure or harm due to COVID-19.
We will offer a 50% refund ONLY as testing supplies have been prepaid for by OccuMed. We will honor a 100% credit to use a test for a future date. This test can be transferred to another person of your choosing.
