RT-PCR Testing:
The RT-PCR, DNA test determines if the individual is infected with SARS-CoV-2 and considered to be able to transmit the disease (a positive test) or is negative for the virus. This test cannot tell whether a person is immune from past infection or has yet to be exposed and is still in danger. This test is required to travel into certain states in New England, like Maine. See state guidelines for up to date information.
COVID-19 PCR Acknowledgment Information:
I understand that the intent of this test is to determine whether I have a current COVID-19 infection.
I understand that this is a voluntary test for asymptomatic persons, and that if I am currently experiencing any symptoms or health ailments that I will contact my healthcare provider.
I understand that the intent of this test is to determine whether I have a current COVID-19 infection. I understand that the test has potential risks associated with it, such as possible discomfort or other complications that may occur during sample collection and possible incorrect test results.
Each test, and its result, is provided for informational purposes only and is not a substitute for the care, medical advice or treatment provided by your physician.
The test results are subject to the same privacy and release restrictions that HIPAA (The Health Insurance Portability and Accountability Act of 1996) places on all medical records. The HIPAA Privacy Rule, protects the privacy of individually identifiable health information. Patients should be aware that test results will not automatically be forwarded to your physician or become a part of office medical records unless you make a specific request. Results will be emailed directly to you, only if the “Authorization to E-mail My Test Results” section of the Intake form is complete.
OccuMed serves as a collection service only. All test’s will be reviewed by medical professionals at ARCpoint Labs.
We will offer a 50% refund ONLY as testing supplies have been prepaid for by OccuMed. We will honor a 100% credit to use a test for a future date. This test can be transferred to another person of your choosing.
Next day results will be administered 36-48hrs from the closing of OccuMed’s Lab on the day of testing.
